Additional Services

Extended Medical Absences/Surgery

If at all possible, students knowing they will be out of school for any period of time should discuss and plan with teachers.

If there is any chance that the student will need any medication at school upon their return, bring Medication Administration Form B to doctor's appointment. 

Elevator access--Mrs. Mathis requires a $5 deposit on elevator keys.  Don't use crutches on the steps.
In the case of surgery or major injury, a physician's note it required with diagnosis and physical limitations.

Special Needs

If your student has a special need related to a medical condition, please contact the school nurse.  An Emergency Action Plan and Individual Health Care Plan can be developed.  If the need is related to asthma, allergies or diabetes, there is a form to complete which can be accessed under Forms.

Allergies

Some students have severe food allergies which can be very serious or life-threatening.  If your student has food allergies, indicate this on the Emergency Medical Authorization Form and complete an Allergy Information Sheet.  We have provided a "Nut Safe" table in the cafeteria.  Most of the food offered in the cafeteria will have allergy information available to students.  If your child has food allergies, please stress that it is imperative that they check labels and in the case that a label is not available, they should refrain from eating an item.  If you have further concerns, contact the school nurse.

Screenings

The state of Ohio mandates that all students in the ninth grade and students new to the district have their vision and hearing screened.  If a problem is detected, the parent/guardian will be notified by a letter.

Immunizations

In order to attend school in Ohio, your student must have completed immunizations required under the Ohio Revised Code.  For an up-to-date list of what immunizations are required, go to http://www.fairborn.k12.oh.us/docs/vaccines%202008.pdf.

Emergencies

An updated Emergency Medical Authorization Form will be kept on each student.  These are mailed prior to the start of school and should be returned on the first day, completed and signed.  If information changes during the year, it is the parent/guardian's responsibility to notify the school nurse.  If a major accident or emergency occurs, the safety and care of the student is the priority and a parent/guardian will be notified as soon as possible (and usually simultaneously).
